For those who enroll in one of the medical plans, Asset Living will utilize Quantum Health for care coordination and healthcare navigation services. Additionally, Quantum's MyQHealth Care Coordinators will now be your primary contact for benefit related questions for all benefits offered by Asset Living.
They are here to simplify your healthcare experience by replacing ID cards, finding in-network providers and so much more. Your Care Coordinators are with you every step of your healthcare journey, from navigating medical claims, check-ups and even pre-certifications. Your Care Coordinators can help organize and simplify your benefits to provide you with a better experience when you need care. When necessary, Quantum Health's nurses, clinicians and benefit specialists are ready to advocate for your healthcare needs.

You do not need to be enrolled in a Quantum health plan to utilize this service!
Lower quality medical care can lead to higher complications, unnecessary procedures and a higher change of misdiagnosis-costing you more time and money.
For those enrolled in the Quantum Health plans – when your providers office asks “Who is your health insurance carrier?" Simply reply your coverage is through Quantum Health with the UnitedHealthcare (UHC) Choice Plus network. When in doubt, providers should contact Quantum's Provider Customer Service at 833-740-3261
Prescription drugs are administered through OptumRx. Your ID card has the needed information for your pharmacy to process your prescription drug claim. When in doubt, pharmacies should contact the OptumRx number on the back of your ID card, or call Quantum member services
