Life & Disability Insurance
Life insurance is an important part of your financial security, especially if others depend on you for support. That's why Clario provides you with Basic Life & Accidental Death and Dismemberment (AD&D) coverage through Prudential, at no cost to you. You also have the option of purchasing Supplemental Life Insurance.
Disability insurance provided by Clario work together to help you pay your household expenses if you become disabled and cannot work. These disability benefits also work with other sources of coverage to replace a certain percentage of your earnings. As a result, the disability payments you receive from our plans will be reduced by any benefits you are eligible to receive from Social Security, Workers' Compensation, Retirement Benefits, or any other disability coverage to which you are entitled. Your disability benefits are administered by Prudential.

Company Paid Life and AD&D
Basic Life and AD&D
Basic Life Insurance pays your beneficiary a lump sum if you die. AD&D (Accidental Death & Dismemberment) coverage provides a benefit to you if you suffer from loss of a limb, speech, sight, or hearing, or to your beneficiary if you have a fatal accident. The cost of coverage is paid in full by the company. You are automatically enrolled in these coverages administered by Prudential.
Clario provides you with an amount that equals two times (2x) your annual earnings, rounded to the next highest $1,000, up to a maximum of $1,000,000. For amounts over $50,000, the premium cost paid by Clario to the insurance carrier becomes taxable income (imputed income) to you.
Portability & Conversion
If you leave Clario, you may be eligible to port or convert your basic and your optional life coverage(s). To be eligible to port coverage, you must have been actively at work on the date employment ended. You must complete an application and apply for these options within 31 days of your coverage termination.

Supplemental Life

FOR CHILDREN**:

*Coverage is subject to reduction beginning at age 70. **Children are eligible from live birth through 26 years of age.

Disability Benefits
Short-Term Disability (STD)
This coverage provided a weekly income replacement benefit that equals 66.67% of your weekly pre-disability earnings (plus commissions, if applicable) up to a maximum of $3,000 per week. Benefits will begin on the eighth calendar day of disability due to an injury/accident or illness. Short-Term Disability benefits are payable for a maximum of 12 weeks. Short-Term Disability Premiums are paid by Clario. Please note: If you work in California, Hawaii, New Jersey, New York, Rhode Island, or the Republic of Puerto Rico, you will receive the applicable state mandated disability benefits. The benefits available under the Prudential Short-Term Disability plan will be reduced by any benefits received from a state mandated disability plan or/and a state leave plan.
Long-Term Disability (LTD)
If you remain disability for 90 calendar days due to any one disabling illness or injury, you are eligible to apply for Long-Term Disability benefits. If approved by Prudential, Long-Term Disability coverage provides a continued source of income that equals 60% of your monthly pre-disability earnings (including commissions, if applicable) to a maximum of $14,000 per month. Long-Term Disability benefits are generally payable to Normal Social Security Retirement Age.
Family and Medical Leave Act (FMLA)
The Federal Family and Medical Leave Act (FMLA) allows up to 12 weeks of unpaid, job-protected leave for specific family emergencies such as a serious illness, birth or adoption of a child and up to 26 weeks to care for a service member who is injured in the line of duty. You are eligible for FMLA leave when you work at a location with 50 ore more employees within a 75-mile radius, have been employed by Clario for at least 12 months and have worked a minimum of 1,250 hours in the prior 12-month period. This benefit is available to you unless otherwise mandated by your state.

There are many state legislation you must follow when you file for disability that it becomes hard to navigate. When you file your claim with Prudential, Prudential will offset your total disability / leave payment by the amount the state is required to pay you.

- Your leave specialist will email confirmation on the claims filing with status updates
- 2 Weeks before your return to work, Sparrow will contact you and see if any forms are needed or if an extension is needed
- Once you return, Sparrow will check in on your transition
How it Works
Please reach out to the HR team at least 30 days in advance to discuss your need for leave or as soon as reasonably possible for unforeseen leaves
Before My Leave
Once you are connected with Sparrow, your Sparrow Leave Specialist will do intake with you to develop your leave plans based on your unique circumstances. This intake will include an overview of Clario’s LOA policy applicable to your leave type, state and/or private disability insurance benefits, and federal, state and local job protection regulations. They’ll help you map out your entire leave and answer any questions that you may have about your leave and wage replacement during your absence .
After intake, Sparrow will provide a recap and create your custom leave plan based on your leave goals. If elected by you, Sparrow will also prepare any claims paperwork on your behalf and be on standby to file them when your leave begins.
During My Leave
If elected, your Sparrow Leave Specialist will file any state or private disability claims on your behalf as soon as your leave starts. For disability claims, Sparrow will also reach out to your healthcare provider to provide them instructions for their portion of the claim. Throughout your leave, you will be notified of the status of your claim and any next steps. Sparrow will always be available to ensure all your questions or concerns are addressed, and will check-in at significant touchpoints during your leave. One to two weeks prior to your return, your Sparrow Leave Specialist will send reminders to the HR team and your manager to prepare for your return.
After My Leave
After your leave has been completed, your Sparrow Leave Specialist will be available to answer any remaining leave-related questions that arise.
